What N494 is really about
N494 is the research course, and for most RNs it is the least familiar work in the program. Eight modules on a monthly start take you from writing a searchable clinical question through database work, study designs, appraisal, and the ethics that govern research on human beings. The reading covers quantitative and qualitative traditions side by side, sampling and measurement, validity and reliability, and the difference between statistical significance and a result that would change anything at the bedside. Most of the lost points in this course come from flattening the evidence. A paper cites a meta-analysis, a single retrospective chart review, a professional opinion column, and a nursing textbook, and treats all four as if they proved the same amount. Sometimes the reverse happens, and a well-designed qualitative study gets waved off as anecdote because it carries no p value. Graders read for whether you know where each source sits and why that position matters to the claim it supports. Name the design in the sentence that introduces the source, keep the hierarchy visible in your evidence table, and stop calling a source strong when what you mean is that you agree with it.

How do I work out what level of evidence a study sits at?
Start with the design, not the title. Ask whether the authors ran a study themselves, pooled other people's studies, described a case, or offered an informed opinion, and whether there was a control group and randomization. That answer places the source. Then say it in your own sentence, so the reader sees that you know where it sits.
Can I appraise a study from its abstract?
Not reliably. An abstract reports conclusions and hides the attrition, the measurement choices, and the limitations that decide whether those conclusions hold. A reference list assembled from abstracts and summary sites falls apart under a grader who opens one article. Get the full text through the library and appraise what the methods section actually says.
The assignment wants best available evidence. Does a qualitative study count?
It does, when the question is about experience, meaning, or why people behave the way they do. The hierarchy is not one ladder for every question. What earns points is saying why the design fits the question you asked, then appraising it on qualitative ground such as credibility and transferability rather than on sample size.
